About The Integration
The integration between Open Health Hub and Sanday Platform enables structured, standards-based data exchange tailored to Dutch general practice workflows. Open Health Hub complements Sanday Platform by supporting patient and professional forms, hybrid care pathways, and collaboration with care partners around the GP practice. Structured data is captured outside the GP system and exchanged securely using FHIR standards. This allows GP practices to use digital intake, monitoring, and coordination with regional partners while Sanday Platform remains the central system for day-to-day clinical work. The result is better-prepared consultations, reduced administrative effort, and smoother collaboration across the care network.
How To Connect This
Open Health Hub works closely with the Sanday team on both technical and functional alignment. This collaboration ensures that the integration fits Sanday’s architecture, GP workflows, and interoperability standards used in Dutch primary care. The connection with Sanday Platform is configured through the Open Health Hub admin and settings environment. Secure FHIR-based connectivity, identity and access controls, and governance settings are set up in a configuration-driven manner, in line with the jointly developed integration approach. This enables GP practices and partners to move quickly from integration activation to practical use cases such as digital questionnaires, hybrid care pathways, and regional data exchange, while maintaining security, compliance, and interoperability standards appropriate for primary care in the Netherlands.
