New ACP Module: Record, Share, and Access Advance Care Planning Directly from the EHR

We are proud to introduce the new ACP Module. Available as an extension to existing EHR environments, the module enables healthcare professionals to record, share, and access Advance Care Planning (ACP) information across the care continuum, directly from the system they use every day. This not only creates a more efficient care process but also provides greater clarity and peace of mind for patients and their loved ones.
A New Step in Digital Support for Advance Care Planning
With the introduction of the ACP Module, we are adding an important building block to the Open Health Hub platform. Alongside our solutions for hybrid care pathways, patient questionnaires, and interoperability, we now offer a unique solution for Advance Care Planning, bringing documentation, data exchange, and workflow integration together in a single module.
Recording ACP Using National Standards
Advance Care Planning starts with consistent documentation. The ACP Module supports registration according to the Dutch IKNL standard for Advance Care Planning, while maintaining the flexibility to add organization-specific fields and additional information whenever needed.
Data is recorded using open standards such as FHIR and ZIB-based information models. This creates a standardized and future-proof approach to documentation, ensuring that information is not only useful within a single organization but can also be exchanged easily with other healthcare professionals involved in a patient's care.
Not Just Recording, but Also Sharing
Advance Care Planning only delivers its full value when information is available at the moment it is needed. For this reason, the ACP Module focuses not only on documentation but also on secure information exchange across the care continuum.
Through FHIR-based interoperability and enterprise-grade security, relevant ACP information can be shared safely and in a standardized way with other healthcare professionals, ensuring that information is available where and when it is needed. The recorded information can then be accessed directly through dashboards and the FHIR Viewer within the EHR. This improves continuity of care and ensures that healthcare professionals have quick access to up-to-date information regarding treatment preferences, goals, and care agreements.
Working from the Familiar EHR
Our implementations and conversations with healthcare professionals consistently highlight the same message: simplicity and integration are essential for successful adoption. Healthcare professionals prefer to work directly from their EHR, without switching between additional portals or standalone applications.
That is why the ACP Module has been developed as an EHR add-on. Healthcare professionals can record, review, and share ACP information without leaving their familiar work environment. By fitting seamlessly into existing workflows, solutions are adopted more quickly and become a sustainable part of daily practice.
More Clarity and Peace of Mind for Patients and Their Loved Ones
At its core, Advance Care Planning is about documenting the wishes, preferences, and goals of patients. By recording this information centrally and making it available to all relevant healthcare professionals, patients and their loved ones no longer need to repeat the same, often emotional, conversations multiple times.
In addition, information and educational pathways can be used to inform patients about Advance Care Planning before these conversations take place, helping them better understand the topic and prepare for the decisions ahead. When preferences or agreements change, updates are immediately visible to all involved healthcare professionals. This reduces misunderstandings, improves transparency, and increases confidence that care aligns with what matters most to the patient.
First Experiences
The first experiences have been gained at the regional level. In addition, we are currently in discussions with several organizations regarding the further rollout of the module. We look forward to working with these organizations to bring the benefits of standardized documentation, interoperability, and integrated workflows into daily practice.
