Improved Home Measurements: Making It Easier to Share Measurements

Patient Education as a Driver for Hybrid Care

Home measurements play an important role in hybrid chronic care. With the improved Home Measurements feature from Open Health Hub, patients can now more easily submit multiple measurements, such as blood pressure and glucose readings. Fewer clicks for patients and more efficient processing for healthcare professionals.

Easily submit multiple measurements

For some measurements, a single reading is not enough. Blood pressure monitoring, for example, may require patients to take measurements over one or several days. The improved Home Measurements feature makes it easier for patients to record and share these readings with their care team.

This simplifies the process and lowers the barrier to taking measurements at home. Healthcare professionals, such as practice nurses, can then use the submitted data to support patient care.

Seamlessly integrated into care workflows

Home Measurements can be incorporated into digital hybrid care pathways, for example, for cardiovascular risk management (CVRM) or type 2 diabetes (DM2). Patients take measurements at home, while the data fits into their existing care workflow.

Measurement data is processed according to applicable coding standards and field definitions and, where supported, flows directly into the EHR. Data that cannot be written directly to the EHR can be accessed through the dashboard integrated into the EHR workflow. This keeps the results accessible to healthcare professionals without unnecessary additional steps.

Greater convenience, more time for better care

The improvements to Home Measurements reflect Open Health Hub’s broader ambition: making digital healthcare fit seamlessly into the daily workflows of patients and healthcare professionals. By reducing unnecessary steps, it becomes easier to involve patients actively in their care and integrate home measurements into everyday practice.

Further improvements are also on the roadmap to make the experience even easier for patients and healthcare professionals, save time, support better care and patient self-management, and enhance healthcare professionals’ job satisfaction.
