Apply smart rules such as conditional logic to create adaptive questions based on given answer options or answer values. Set minimum and maximum values and step size for number and slider questions. Even add a formula module, so that patients can see their intermediate subscores while filling our a medical form. Or more advanced, specify export labels that will be used for the data that is exported with a FHIR client.

Integrate subscores & code systems

Add calculations to create your own subscores and link these with code systems. For example, create a subscore for Quality of Life based on a calculation of four separate PROM questions. Then, specify how this subscore relates to which code in your code system, whether they are internationally recognized code systems such as SNOMED or LOINC, or your own local code systems within your EHR.

Integrate code systems

Integrate your code systems – such as SNOMED – directly into our forms. By incorporating standardised codes and medical terminologies, you’ll enhance the efficiency of your data collection process by directly translating responses into the right medical codes within your EHR.

Conditional rules

Easily set conditional rules so that patients only see those questions that are relevant to them. Decide which question is shown next based on either the chosen answer option or based on a (calculated) value.

Scores & subscores

Use scores and subscores for more detailed and specific measurements. This way, you can measure an area such as anxiety based on multiple questions, while mapping certain lifestyle factors with an intake form.

Scheduling & notifications

Set the timeline of any content step. For example, automatically send an intake form and PROM questionnaire after a new patient signs up. Repeat the PROM at intervals you choose, and conclude with a PREM form to measure patient experience. Then, specify which steps are mandatory and whether and when patients receive notifications and reminders.
