Design, Collect, View: how Open Health Hub simplifies patient data collection

Many healthcare organisations struggle with collecting and viewing patient data. Limited by the constraints of healthcare information systems, such as their EHR, they often work with non-editable medical forms to collect patient data, and are restricted in the way they can visualise patient data within their EHR.

Frustrated by limited or costly customisation, they turn to point solutions, only solving a part of the problem. Open Health Hub offers a FHIR-based platform for medical forms that enables healthcare organisations to design their medical forms in a quick and easy way, collect patient data through our platform or fully embedded within their existing systems, and view patient data in a custom dashboard right within their EHR or other healthcare information systems.

Design

Challenges

  • Healthcare organisations are not able to easily edit existing medical forms. And even if this is possible, it is very costly and time-consuming because they often need to negotiate with their EHR- or medical form provider. There is a high dependency on and lock-in with these EHR providers and other healthcare partners. 
  • Designing custom or rare medical forms is time consuming and costly. There often is no library available for these forms, so healthcare organisations need to design them themselves.
  • There is currently no easy and friendly to use form designer. Vendors often ingnore or are not willing to help here

How it should be

We think designing and editing medical forms shouldn’t be this hard. The healthcare industry would benefit from easier and cheaper medical form customisation, savings time and costs, with more efficient patient care as a result. 

How we do it

We have developed a Design solution that makes it possible to design medical forms in a super easy way. Choose from our extensive library of validated medical forms or design your own from scratch. Change an existing form? No worries, healthcare organisations easily edit questions or add a new one.

Features

  • Integrate code systems: our Designer gives healthcare organisations the power to manage and integrate code systems – such as SNOMED – directly into their forms. By incorporating standardised codes and medical terminologies you’ll enhance the efficiency and effectiveness of the patient data collection process and patient data exchange. It ensures that health data is findable, accessible, interoperable, and reusable, aligning well with the FAIR principles and ultimately contributing to improved patient care and health outcomes.. 
  • Conditional rules: with conditional rules you create questions that adapt based on the patient’s form responses. Add logic to your forms to save your patient or candidate time and increase the response rate.
  • Many question types: choose from a wide range of question types such as a slider, multiple choice, multiple select, a date questions, or add a photo question so patients can take a picture of a wound or their medication.
  • Scores and subscores: use scores and subscores for more detailed and specific measurements. This way, healthcare organisations can assess areas like anxiety and depression individually for mental health measurements, while mapping certain lifestyle factors with an intake form.

Collect

Challenges

  • Healthcare organisations often collect patient data via a standalone point-solution, developed to do just that and nothing else and which doesn’t integrate easily with other systems.
  • These point solutions are don’t work well across healthcare departments and different types of healthcare organisations.
  • The patient data that is collected with these point solutions is shared with healthcare organisations in an inefficient way and format. Sometimes they must still manually enter this information into other systems or they receive a simple CSV that can be exported. 
  • Privacy and security are a big challenge in collecting patient data. 

How it should be

We think collecting patient data should be safe and secure up to the highest standards and done according to the FHIR standard (Fast Healthcare Interoperability Resources) for interoperable use, so that patient data can be directly shared across other healthcare information systems via end-to-end encryption. 

How we do it

We make it easy for healthcare organisations to collect patient data by sharing our medical forms with patients right within their existing systems, completely white-label. We collect patient data safely, securely and compliant with the latest ISO and NEN certifications and HIPAA regulations and fully end-to-end encrypted. Our Collect solution is built based on FHIR, meaning that collected patient data is structured in an interoperable format for use across other systems, or even for AI applications.

View 

Challenges

  • Healthcare organisations are currently significantly limited in the ways they can visualise patient data within their EHR or other healthcare information systems. 
  • Combining patient information from various data sources within one system such as the EHR is impossible due to different formatting of patient data. Healthcare professionals often need to switch between different healthcare information systems to build a complete picture of the patient. This is time-consuming and unnecessarily delays crucial clinical decision-making.
  • Many EHRs are built on legacy software and are not designed according to the FHIR standard. 

How it should be

We think building a holistic and comprehensive view of the patient’s health status can be easier. We think it should be possible to have one place where healthcare professionals can access and view all the collected patient data and build a dashboard to get the insights that they need. 

How we do it

Patient data captured through our medical form solutions is structured according to the FHIR interoperability standard, meaning that the patient data can be readily shared and viewed within other systems without manual data entry, such as within our own Viewer. We think all patient data collection solutions will move towards FHIR in the future.
